Since Risk Response is already in place appropriate cost/schedule buffers are added to the plan. So cost/schedule baseline already has it included. So no CR is needed.
Should the response be unsatisfactory to cover the risk and further response will affect any baseline, a change request is required.
Perhaps a better, more broad way of thinking about it is that if anything happens to a project (or you're going to do something to a project) once the project is in motion, a change request is necessary.
But what if the implemented response (the 1st planned response not a 2nd or further response) will affect parts of the baselines that weren't affected before implementing the response, do we still need to submit a change request?

Implementing a response to an identified (and therefore planned) risk does not require a change request. However, if you find your response is unsatisfactory and further response will require a change in one of the baselines, then a change request is warranted.

When implementing a risk response as a reaction to an identified trigger, do we still need to submit a change request? Even though the appropriate contingency reserve was previously assigned for such risk?
