TOPIC: Effort breakdown per Process group for PMP application

Hi, I am filling out the work experience section of my PMP application. I do remember the duration of the projects I have managed.
But I can't get hold of the project schedules.
May I request a template, a suggested format, or % breakdown that will help me break down my effort per process group. Thanks!

We don't have such a template any longer because PMI changes their online application form so often that it is impossible to keep it updated. So we stopped years ago.
