If a situation in a projects has an issue which needs to be resolved by creating a change request then what should the PM do first, if both of the following choices are given?
1. Update issue log
2. Create change request.

Sushil,

Based on your description "a project has an issue which needs to be resolved by creating a change request" the order seems clear:

1. An issue was identified and added to the issue log
2. Upon review/analysis of the issue, it was determined that a change request should be created in order to resolve the issue
